 Today the hospital encourages employees to report mistakes, and also to report issues that might lead to mistakes. A committee of peers reviews serious incidents. If the panel determines that a mistake has occurred, the hospital and doctors apologize. In addition, the University of Michigan compensates the patient or the family. The costs to such a review are high, said Mr. Boothman. They are included in the hospital's risk-management budget, which has grown to more than $3 million annually, up from $500,000 in 2001. But claims against the hospital have been reduced to 106 in 2008 from 121 in 2001. Andrew Muth, an Ypsilanti, Mich., plaintiffs lawyer, said the hospital's project achieves the primary goal of litigation: getting to the truth quickly and efficiently. In the second initiative, plaintiffs lawyers in participating states must retain medical experts who have practiced in the same specialty area as the physicians accused of malpractice.

 In fact, according to the Electronic Privacy and Information Center, federal agencies have negotiated agreements and contracts with social networking sites like Google, YouTube, SlideShare, Facebook, AddThis, Blist, Flickr and VIMEO to collect information on visitors for federal websites. All of these private companies are known to have agreements with federal agencies, but the public has never seen them. In public comments submitted to the Office of Management and Budget, EPIC notes it has obtained documents that show federal agencies have negotiated these contracts with the private sector in violation of "existing statutory privacy rights." Those agencies include: Department of Defense, Department of the Treasury, and the National Security Agency. There are suspicions the White House is already involved. According to Obama "technology czar" Vivek Kundra, the "compelling need" driving this major policy reversal is the administration's desire to create "more open" government

 Everyone knows there will be incompetence found in any large organization. We all know ‘upper’ management despises any appearance of incompetence and/or weakness, hence the inevitable ploy known as the “cover up”. Even though these cover ups are nothing new, I still am amazed at the depths of a military cover up. The loss of military equipment, particularly weapons and, OMG, hydrogen bombs, presents a grave concern to the general public. The fact that the military would prefer to cover up these losses and stop looking for them in favor of preserving the outward appearance that they are trustworthy and dependable to keep us all safe, is nothing short of criminal. In light of all the known cover ups we have been forced to endure over the years, is there any possible way that the American public can ever trust politicians, military personnel, CEO’s, bankers, policemen, or any other person of authority?

 I don't care which party it is or who it might be. This thing ('stimulus package') is so huge that it would take an army of monitors to maintain true accountability. I think many Americans might support the efforts behind the 'package' if they truly believed that it wasn't throwing good money after bad and that bad investments, bad management, and pure corruption were not going to be the main profiteers of all our money. As to the idea that Republicans want the project to fail so they can look good...well, I find that highly offensive. I, for one, do not. I think it was a stupid idea. Even the current Administration acknowledges it doesn't know what all's in it as does the very Congress responsible for it. Only stupid people sign on the dotted line without knowing the obligations it will entail.

Partisan politics played a part. "To delay getting our funds would be a political disaster for the President," a lobbyist wrote to the Office of Management and Budget in 2003. "His election will hang for a large part on show of progress in Iraq and without the funding this year, progress will grind to a halt." Remember, if you're a neocon, every problem is a public relations problem. They're solved with PR, propaganda, and Potemkin villages.

Commercial banks such as Bank of America and Mellon Bank, seeing the opportunity to collect management fees on the accounts, jumped into the business. "Every bank wants to increase its share of HSAs," said John Casillas, director of the Medical Banking Project, a Franklin, Tenn., organization that helps medical administrators develop financial service systems. "There's fees for managing the account, transaction fees, fees for investing the funds," Casillas said. "You're going to see many billions of dollars moving from premium payments to professionally managed investment funds under HSA rules. Some people think that banks are going to threaten health plans by replacing them in the marketplace.""

  Five of the leased wells found natural gas three decades ago August 20, 2008 Lautenberg and Menendez top 'porkers' New Jersey Sens. Frank Lautenberg and Bob Menendez were the No. 1 and No. 2 "top porkers" cited by Citizens Against Government Waste this week for the targeted spending items they sponsored in a transportation and housing bill approved last month by the Senate Appropriations Committee. CAGW said Lautenberg sponsored $85.4 million in earmarks and Menendez $83.9 million, but that could be misleading because Lautenberg cosponsored all of Menendez's items, so you can't add the two numbers together. In other words, the total outlay to New Jersey is $85.4 million -- and $75 million of that is for one project -- a new rail tunnel under the Hudson River. CAGW didn't list the items it considered pork, and it never does. He argues that it could be made workable by locating it in regions that have a combination of low-cost 'stranded' energy considered too remote to be economically viable to exploit – like flared natural gas or solar energy in deserts – and that are rich in limestone, making it feasible for calcination to take place on site.
