Medical miscommunication is a large problem in the medical field lately. But why is it happening more and more often?

Based upon personal experience I would say that one of the reasons for miscommunication is the fact that doctors more often work with checklists to determine a patients condition. Bloodpressure? Check! Temperature? Check! Bloodcount? Check!
This improves efficiency in the short run, but by building on those 'facts' in the longrun more and more vital information will be missed. Some of this input may turn out to be vital.
Besides that I think that doctors in general do not know how to interact with patients who not only want their expertise, but are also searching the net for information about their disease. Patients on the other hand are not allways capable of handling the information they f...
